The
therapeutic sessions are confidential to Counselling Team Ltd, and my
GP. The counsellor works with my GP as part of the primary care team,
and may speak with my GP from time to time about my progress. This
usually does not require disclosure about my story.
That CORE and the IAPT Minimum Data set outcome measures are used,
involving my completing a series of computer based questions at every
session with Counselling Team Ltd.
That the information from CORE and the IAPT Minimum Data Set will be
treated as strictly confidential. No one other than Counselling Team
Ltd, the researchers, my GP and me will have access to my responses. If
I am referred to another mental health/psychological service provider
my notes may be shared with them in my best interests.
Any data used for research and or training purposes will be anonymous and I will not be identified.
That a summary graph of the CORE and the IAPT Minimum Data set clinical
outcomes relating to me will be sent to my GP and will be kept as part
of my medical records.
Minimal essential case notes will be kept by Counselling Team Ltd on CORENET the computer information management system.
I understand that the purpose of my therapeutic contract with
Counselling Team Ltd is for my therapeutic benefit in helping me to
improve my quality of life, and not for legal matters or insurance
claims.
I understand that the counsellors are not qualified or authorised to
give a diagnosis, and are not considered to be expert witnesses in a
court of law.
If I require a professional diagnosis for purposes of law or insurance
claim, a full assessment by a clinical psychologist or psychiatrist will
be required. This can be arranged via my GP.
If I fail to keep agreed appointments, - or do not give Counselling
Team Ltd reasonable notice of cancelled appointments, this will result
in withdrawal of the service.
